# GitHub Copilot Instructions for MedLens+

## Mission

Build MedLens+ as a safety-first, longitudinal medical report intelligence platform.

## Non-Negotiables

- Never diagnose disease.
- Never prescribe medicine.
- Never suggest dosage changes.
- Never replace doctor consultation.
- Always use report-provided reference ranges.
- Warn when comparing values from different labs.

## Engineering Defaults

- Use typed contracts and schema-first development.
- Keep provider integrations behind interfaces and factories.
- Keep config env-backed and feature-flag-backed.
- Add tests for any changed behavior.
- Update docs in the same PR for architecture/config/flow changes.

## Branch and PR Rules

- Trunk-based: branch from `main`, PR back to `main` (squash merge).
- Long-lived branches: `main` only.
- All merges to protected branches are **manual PR merges** — auto-merge disabled.
- **Every commit message must pass `commitlint.config.cjs`** or the required CI check fails the PR: conventional `<type>(<scope>): <subject>` header, valid type, header ≤100 chars, blank lines between header/body/footer. `body-max-line-length` is disabled (commit bodies routinely contain unwrappable URLs).
- Link a GitHub issue (`Closes #NNN` or `Relates to #NNN`) in every PR.
- Use the [PR template](../pull_request_template.md) — summary, safety impact, privacy impact, test evidence.
- **AI reviews (Copilot, CodeRabbit) are manual only** — request when the PR is ready and scoped (see `docs/07-open-source/08-TOOLING_SETUP.md`).
- **PR size (recommended):** ≤150 changed files for AI tools; ≤400 lines meaningful diff; one logical change per PR when possible.

## Local Validation

Run before requesting review:

- `pnpm lint`
- `pnpm typecheck`
- `pnpm test`
- `pnpm build`
